Prior to the grant application opening, you can download the Medical Evaluation Form to fill out and submit to your licensed healthcare provider.
The Medical Evaluation Form must be completed within one year of applying.

Eligibility Checklist

Use the following criteria to determine your eligibility:

  • Infertility Diagnosis: Defined by ASRM, “The need for medical intervention, including, but not limited to, the use of donor gametes or donor embryos in order to achieve a successful pregnancy either as an individual or with a partner.”
  • Relationship Status: Applicants relationship status does not impact the grant selection.
  • Health: Applicants must demonstrate both physical and mental well-being via the Medical Evaluation Form.
  • Insurance: Applicants must hold basic medical insurance for mother and baby, including prenatal coverage.
  • Residence: Applicants must be permanent residents of the United States and must be under the care of an accredited fertility clinic (of their choice) within the United States.
  • Living Arrangements: All applicants must maintain adequate living arrangements (must lease or own).
  • Child Support: Applicants must provide continued financial support of the child.
  • Treatment Timeline: Grant recipients must utilize funds within 12 months of receiving a grant.
  • Previous Treatment: Hopeful Mama Foundation does not reimburse for previous treatment.
  • Financial Grants: Applicants MAY NOT apply if they have received financial assistance from other nonprofits/organizations. This DOES NOT include discounts through fertility clinics/pharmacies.
  • Services Covered: The services covered are at the discretion of the Board of Directors and may include, but not limited to, monitoring, lab work, medication, egg donation, IUI, IVF, and gestational surrogacy.
