The Hopeful Mama Grant supports various paths to parenthood, including fertility treatments, donor conception, surrogacy, and fertility preservation for eligible medical conditions—empowering recipients to pursue their dreams of motherhood.

To be eligible for this grant, applicants must meet the following criteria:
- Must be a woman with a formal medical diagnosis of infertility, supported by documentation from a licensed healthcare provider.
- Must be a US citizen or permanent resident.
- Must be seeking treatment at an accredited fertility clinic within the United States.
- Must demonstrate financial need and provide details regarding the anticipated costs of treatments or processes related to achieving parenthood.
- Infertility Diagnosis: The necessity of medical assistance to achieve a successful pregnancy, either alone or with a partner. Infertility Diagnosis must be a medical diagnosis determined by a healthcare provider.
- Health: Applicants must submit The Hopeful Mama Foundation Grant Medical Evaluation Form related to physical and mental well-being.
- Insurance: Applicants must hold basic medical insurance for mother and baby, including prenatal coverage. Applicants are not required to have fertility coverage.
- Living Arrangements: All applicants must maintain adequate living arrangements.
- Ongoing Financial Responsibility: Applicants must demonstrate the financial ability to support the child.
- Treatment Timeline: Grant recipients must utilize funds within 12 months of receiving a grant, unless otherwise approved by the Hopeful Mama Foundation Board of Directors.
- Previous Treatment: Hopeful Mama Foundation does not reimburse for previous treatment.
The following expenses may be eligible for coverage under the Hopeful Mama Foundation Grant as determined by the Board of Directors:
- Procedures, such as SIS, SHG, hysteroscopy, and polypectomy to prepare for fertility treatments.
- Fertility treatments such as medicated cycles, intrauterine insemination (IUI), in vitro fertilization (IVF), including egg retrieval, fertilization, and embryo transfer.
- Universal genetic carrier screening and preimplantation genetic testing (PGT).
- Costs for donor egg(s) and/or donor embryo(s).
- Costs for surrogacy.
- Fertility preservation procedures, such as egg retrieval for freezing, for medical diagnoses that may impact future fertility, including:
- Cancer treatments that could impair fertility.
- Premature Ovarian Insufficiency (POI) or other medical conditions affecting fertility at a later age.
The following expenses are not eligible for coverage under the Hopeful Mama Foundation Grant:
- Any costs incurred prior to grant approval.
- Travel, lodging, or meal expenses related to treatments.
- Embryo storage fees.
- Medications or procedures not prescribed directly by the fertility clinic.
- Appointments or expenses after the recipient has graduated from their fertility clinic.
- Expenses related to additional transfers or treatment attempts beyond an initial successful live birth supported by the grant.
- Note: Each grant term covers one successful live birth. Any future treatment attempts after a live birth will not be included in the grant term.
Applications will be evaluated using a scoring rubric.The scoring process is designed to prioritize applicants with the greatest need, ensuring that the available funds are distributed fairly and effectively.
The Hopeful Mama Foundation Board of Directors has the final authority to approve grant recipients and disbursement of grant funds. Decisions made by The Hopeful Mama Foundation Board of Directors are final and cannot be appealed.
The grant amounts may vary among recipients, and partial and full grants can be awarded. Funds from the grant will be disbursed directly to the recipient’s clinic and/or pharmacy. If you receive a grant, the Hopeful Mama Foundation Board of Directors will determine the value of the grant based on the Foundation’s available funds and the applicant’s need determined by the grant rubric. While the Hopeful Mama Foundation would love the ability to offer grants to every applicant, not all applicants will receive grants.

The Hopeful Mama Foundation Board of Directors reviews applications. Grant recipients will be notified by phone approximately 4-6 weeks after the grant deadline of May 10, 2025. Those not selected will be notified via email.
To protect applicants’ privacy, information for those not selected will be destroyed within 30 days of grant awards.
The Hopeful Mama Foundation reserves the right to request additional information during the review process.

Application Checklist
To apply for a Hopeful Mama Foundation Grant, applicants must submit the following:
- Completed Hopeful Mama Foundation Grant online application
- Upload a written history of your fertility journey (maximum 1,000 words)
- Upload a written statement explaining why you chose to apply for a Hopeful Mama Foundation grant (maximum 500 words)
- Upload the Hopeful Mama Grant Medical Form: This form must be completed by both you and your licensed healthcare provider (located at the top of the page)
- Upload images of both the front and back of your insurance card(s)
- Signed media release granting the Hopeful Mama Foundation permission to share your story (included in this application)
